Get the N'tormat in Risk of Rain 2

Start a Risk of Rain 2 run and get the N'tormat. It comes from a Wake event or a Trial, and Wake events spawn at random.

A Trial gets you the N'tormat on demand. Go to the Bazaar Between Time, jump down the giant hole, and find the gong on the ledge. Hit it to open a portal and start the Trial. Take the portal, complete the Trial, and you receive the N'tormat and one page.

Find the Page of Reincarnation

The door needs one specific page, Reincarnation. It unlocks the Reincarnation ritual inside the N'tormat. The page from your first Trial may not be it, so keep collecting.

Trials, Wake events, and Shrines of Compulsion all give pages. A Shrine of Compulsion asks you to finish a task, and the reward is a page.

Pages can also spawn near N'kuhana's altar, which sits in a secret tunnel beneath Wetland Aspect. The Page of Reincarnation reportedly shows up there too, though it's random.
